iHealthHome Launches Innovative Care Planning Tool to Enable Seniors to Thrive at Home

NOVEMBER 2017

iHealthHome’s closed loop plan of care powers the meaningful inclusion of family members, caregivers, technologies and seniors as part of the care team.

Software vendor iHealthHome announced today a major release of their Interactive Plan of Care solution that empowers professional Care Managers with the digital tools they need to coordinate care and leverage in-home technologies that help seniors stay healthy and independent at home.

Navigating the complicated healthcare and eldercare systems to meet the goals for a senior at home requires a comprehensive and holistic blueprint of the many support resources that must be coordinated. This blueprint is called the Plan of Care. The needs of each senior at home are unique and ever changing and the Plan of Care should be just as dynamic and evolving. With iHealthHome, Care Managers are now powered by a tool that not just outlines the plan, but brings it to life with seamless communication and interactivity through mobile and telemonitoring devices.

“What makes our Plan of Care unique is that it truly allows Care Managers to meaningfully include family members, caregivers and even the client to become active members of the care team” states Dew-Anne Langcaon, Founder and Chief Executive Officer of iHealthHome. “We believe in integrating caregiver tasking with biometric telehealth monitoring with community resource management all within the context of the senior’s goals, and we have worked very closely with highly experienced community care managers to develop this closed loop approach to care planning.”

Introducing proactive technology enabled care processes into the home encourages better adherence to the Plan by seniors, improves accountability of caregivers, gives family members peace of mind, and provides physicians with important information about the senior a home to supplement and enhance their medical decision making all of which improves the quality of care at home and reduce the cost burden of aging in place.
