About Us
Our iHealthHome platform was designed by care managers for care managers. We know the challenges involved in coordinating home and community-based care and services and our founders set out to leverage technology and automation to encircle a senior at home with a connected virtual village. We were early adopters of technology starting in 2008 and have seen many changes in the care of seniors at home and have evolved our system to support care managers with the tools they need to offer holistic, comprehensive and coordinated care for seniors who want to stay healthy and independent at home.”
Our story began in Honolulu, Hawaii where co-founders, former hospital administrators, Dew-Anne Langcaon and Bonnie Castonguay started a senior care navigation service called Ho’okele. Ho’okele means navigator in Hawaiian and is the revered role on a voyaging canoe held by one who navigates using the stars, ocean currents and other natural elements to guide ancient voyagers across the Pacific. For families navigating the complexities of senior health care, it can often feel like a stormy voyage with no help to guide you. Inclusion of family or ohana, is another value that drove our founders to find ways to better connect the families of their clients to be part of the care team.
However, helping seniors stay healthy and safe at home is still a highly labor-intensive, manual and paper-based endeavor and therefore very costly. We knew that infusing technology would be a necessary component for transforming the delivery of senior services at home, but the challenge was not the lack of technology, but the workflow barriers to adoption of automation by the very service providers who need it. So we focused on making technology easy to use by in-home providers of care. In 2016, we brought on board software experts in Seattle, WA to bring our vision to reality resulting in the iHealthHome platform that is in use today.
Today, we continue to evolve with the needs of seniors. In 2020, our founders pioneered an innovative new approach to senior home care called Vivia and iHealthHome joined the Vivia Cares, Inc. family of companies. Vivia Cares’ mission is to enable affordable aging well at home. The values of our founders continue to guide us and Our Team is dedicated to making it easy for senior care providers to go paperless, be more efficient, support and supervise their caregivers in clients’ homes, better communicate with the entire care team the tasks that need to be performed based on a holistic plan of care, and so much more.

Meet the Team
Dew-Anne Langcaon
Co-Founder with over 25 years of healthcare leadership experience in healthcare finance, hospital and physician group operations, and strategic planning. Eight years as an owner operator of a private home care agency and care navigation service. Prior to co-founding iHealthHome, served as EVP and Chief Strategy Officer for Hawaii’s largest hospital system, Hawaii Pacific Health.
Dan Fredericks
Co-Founder and principle architect of the iHealthHome software solution. Dan is an experienced business intelligence solution architect with an extensive background working with and managing all areas of technology with a focus on designing and developing various data warehouse strategies. Prior to joining iHealthHome, Dan was a lead architect at TIBCO.
Kara Mayeda
Over 20 years specializing in customer service in the retail and senior care sectors. Prior to joining iHealthHome, Kara served as the Director of Home and Community Based Services with Keiro Northwest in Seattle where she started, operated, and grew their private home care business. Using iHealthHome’s software, Kara implemented a fully paperless process that enabled rapid growth of the agency across the state of Washington.
Tanya Fernandez, JD
Over 13 years of experience in finance, legal, and operations in the entertainment and legal sectors. Tanya is also an experienced operator of a large private duty home care agency customer of iHealthHome. Prior to joining iHealthHome, Tanya served as a tax manager with Deloitte & Touche, specializing in multi-state tax services.
Bonnie Castonguay, RN
Bonnie is a Registered Nurse with over 30 years of experience in healthcare. She is a Certified Professional in Healthcare Quality by the National Association for Healthcare Quality and a Certified Geriatric Care Manager. More than eight years as an owner operator of a private home care agency and care navigation service. Prior to co-founding iHealthHome, served as Vice President of Patient Safety and Quality for Hawaii Pacific Health.
Meet the Board
Dew-Anne is the Co-Founder and CEO of iHealthHome. Dew-Anne is also the Co-Founder and CEO of Ho’okele Health Innovations, LLC (HHI) the majority preferred shareholder of iHealthHome, Inc. Prior to co-founding HHI and iHealthHome, Dew-Anne served as the EVP and Chief Strategy Officer for Hawaii Pacific Health, Hawaii’s largest healthcare system. After serving as a hospital executive for more than 12 years, Dew-Anne became an entrepreneur and started HHI in 2006 out of a passion to help families of seniors navigate the complicated healthcare system and remain healthy at home. What started as a boutique professional service company quickly grew to become one of the largest home care agencies in Honolulu that created a software system for their own needs and in 2014 began its transformation into a salable system to other agencies and is called iHealthHome. Dew-Anne is trained as a certified public accountant, but is no longer in public practice. She worked for Deloitte and Touche for six years. Dew-Anne holds a BBA and MAcc from the University of Hawaii at Manoa. She also serves as the Chairman of the Board for the St. Andrew’s Schools and has previously served on the boards of the American Red Cross, Aloha United Way, Catholic Charities of Hawaii and numerous other community non-profit organizations.
Ben is currently the Chief Financial Officer for Pacific Shipyards International, LLC. His previous experience includes working as a controller for a medical device start-up company, at Servco Pacific Inc., one of Hawaii’s largest privaty companies with distribution and retail automotive operations, as a real estate developer, and as an audit manager for Deloitte and Touche in Hawaii. Ben has a BBA in Accounting from the University of Hawaii and an MBA from the University of Southern California. He is a certified public accountant and is the Treasurer for the McKinley High School Foundation and the past Chairman of the Board for Junior Achievement of Hawaii. Ben is an investor in iHealthHome and has served as a Director on iHealthHome’s Board since 2014. He is also an investor in Ho’okele Health Innovations, LLC since 2010.
Fred is a physician specializing for over 20 years in aesthetic and cosmetic surgery and is the Medical Director of The Stern Center for Aesthetic Surgey in Bellevue Washington. Fred also has held several leadership positions in healthcare including Chief of Ophthalmology at Virginia Mason, Member of the Board of Virginia Mason, Member of the Board for Virginia Mason Health Plans, and Member of the Board for Alliant Health Plan. Fred holds a BA from Tufts University, an MD from Columbia University and completed his residency at the University of Washington. He has been voted “Best Plastic Surgeon” by both King 5 Best of Western Washington and 425 Magazine. Fred’s passion for fine food and wine led him to become an entrepreneur and co-founder of Noviello Vineyards. Fred is an iHealthHome investor and has served on the Board since 2019.
